Hi, IAm Jitu. Iam completely stuck with an issue. In my passport, surname field is blank. My husband has got a H1-B visa, and he will be doing his stamping in January. When we enquired, it was told that for dependent visa, surname field is NON-mandatory. Hence, we planned NOT to change it and get it stamped. Now, I have received a H1-B visa initiation request from my current employer....

Yes, the stamping would be valid, and you have to carry two passports always. But it's ALWAYS better to get name corrected and go for stamping. Your name on the visa will be used for all purposes (like Tax ID number) in USA. Search for other similar posts on this forum, you will find more details.

Hi , As already mentioned that you have to choose "NO" for the question as you do not fall under the Visa RENEWAL category. There is nothing to worry about it , this option is just to filter out the Appointment Dates i.e. sometimes for Visa renewal category you will find some dates open and if you select "NO' , you will find same date as CLOSED. As far as Documents /Check list is ...

Did that H1B come with a new I-94 attached to it? If Yes, then your status is H1B starting Oct 1st 2009. Your employment in OPT status after Oct 1st is illegal and you are out of status if you don't have pay slips from the H1B employer.
