My back used to bother me prior to my T2 diagnosis 6 months ago. But it was always lower back pain, right above the hip and close to the bottom of the spine. I have so many friends with back pain. I think a lot of it is due to poor posture and poor sleeping positions. I do a lot of yoga and had one yoga teacher tell us that when you are sitting properly, you open up your lungs and let more oxygen in. Something has just occured to me.... if your brother doesn't look after himself and he develops long-term complications (problems with his circulation, kidneys, liver, nervous system) he may also experience sexual dysfunction. You could scare him a bit by telling him that he's on course to become impotent within years....
