My DS turned 3 in July. And he had been on a nebulizer 2X a day until March of this year when his doctor changed his medicines. Before that he would be on the nebulizer while DD cleaned up toys. Hey everyone, So my PCP is very willing to write a letter of medical necessity for me. But he has no clue where to start ot what to put in it so he asked for an example letter from the surgeon. The surgeon said they can't do that. I tried googling some example letters and I'm not finding any. Is anyone willing to share a copy of theirs? I'd really appreciate it. I already know...

letter of medical necessity

I need to get Eve a new wheelchair and hopefully a positioning chair. Our new pediatrician has never had a patient need a wheelchair and doesn't know what to write. Is this something the PT should write up for the Dr. to sign? Alternately, are there examples of letters of medical necessity out there (and could there be a sticky for this)?
