lets get some good information together here about spec clutch failures.
lets keep discussion out of this thread and post only the facts.
post what clutch you have, how many miles where on it when it failed, was it making any noises, etc. post any pictures you may have taken of the clutch also.
So I will start clutch kit: Stage 3+ engine: 3L turbo installed: ~March 14, 2008...

Noises: last 2k miles i heard metal scraping while slowly taking off to accerlate. Mostly like a moosing noise.
Symptoms: couldnt shift into gear at all while engine was on. Was able to put it in gear while engine was off.
Reason for failure: .... have no clue... I was rarely ever hard on the engine. ...

Another Spec Clutch threw out on me today
This time it was a normal stage 3.
I got the clutch used and was still in good shape.
Put 4,500 miles on it tore up.
I will post pics as soon as i get it out.

Seems to be the common problem.. nope, you can't just go welding. we don't know if say the heat treat was missed but welding could cause the metal in the clutch hub to become more brittle.

Just wondering, is there any preventative measures to be taken to "rig fix" the problem? Weld a support bracket between the 2 spring "retainers"? Seems to be the common problem..
I would think welding would work. I mean wouldn't you be able to apply pre and post heat treatment to the heat effected zone to prevent imbrittlement and cracking? It would be a complex weld for sure but I would think it could be done. Though I suppose it really depends on what the clutch hub is made of.
