You must be talking about MyWi not MiWi and it comes with a 10 day trial.

You don't need an app, you can enable native tethering on 3.1.2.

Has anyone had AT&T charge them extra tethering fees?

i have been checking my useage and my incoming went from 2.0 gb to 2.1 since i began using it. i used it quite a bit yesteday along with mywi. ill let you know when my bill comes up here soon.

I monitor for that Kaba, and so far, no one here has claimed extra charges for tethering. Disclaimer: That is unless I missed the thread, which is entirely possible.
