  The most popular opinion is reincarnation, they are recycled back into the human world. It's supposed to be an idea based off of Buddhism.

  I think they do get reincarnated in the human world. In the Bleach movie Memories of Nobody, Senna is sort of reincarnated and is seen at the end of the movie as a school girl.      

When a soul in the soul society dies or gets killed (doesn't matter if it's a soul reaper or not), they return to the living world. All memories of their past disappear. This is believed to be why Ichigo resembles Kaien so much. This is how the balance is kept. This is what was said by Tite Kubo as well.
