Yes, it is a sign of pain, or discomfort. I don't think it reduces pain, but i don't know for sure about that.

If the injury is soft tissue, it won't show up on an x-ray. If the injury is soft tissue, sometimes, it can feel worse than a break, and it take a long time to heal. The anti-inflammatory drugs should be helping. Best to leash walk, and keep him as quiet as possible too. Maybe ask the vet to prescribe Valium, as it is, believe it or not, a muscle relaxer and a sedative ... Might help him...

My friend's toy poodle has this problem. Basically she was told by the vet that he needs surgery to correct the problem. The skin is too tight to let it release and go back to normal... She has not had to do the surgery as when it happens she lubricates it and it's able to retract. It's not a normal condition by any means but I think it can be managed without surgery....
