I'd learn (x)html first then css. the javascript and php/myql will depend
on which you'll have need of more. They'll also take a lot longer to really
learn so you'll probably
bounce back and forth between the two.

I doubt most employers would care whether you have the certificate or not. They'll be more concerned with whether or not you can do the work.
Having said that any kind of certificate likely looks good on a resume, especially if you have a lack of experience to talk to. It can't hurt to have the certificate, but you probably don't need it.

How will you upload the file without the form?
Easy way to test it simply comment that line out and see if it works. Do not
