Merchant accounts are generally shut down if the total amount of transactions per month is higher than the preset limit given by the merchant account provider. In this way merchants are unable to process large amount of transactions every month. This not only hampers the growth of the business but it might also lead to customer and potential sales loss.

I see it as a leverage to bring down pricing from international associations, rather than a viable business solution. They may (and most certainly will) succeed in their respective countries, but these cards most likely will NOT be accepted internationally. When it comes to eCommerce related stuff, leave it to the experts.
I don't know of anyone that does everything but you can get all of the pieces pretty easily yourself. Here is my suggestions for a fairly simple setup
Merchant Account - Paypal (Website Payments Pro, probably)
Domain Names - Namecheap (or with the registrar of your choice)

Since you will be responsible for the transaction processed through the processor / Acquiring bank then it is a MUST that this information is submitted to them.
The reasons are many, for instant if you should decide to leave your business without closing your account and for that reason chargebacks will hit the account then they will need to be able to get a hold on you.

Continually adding and removing Credit & Debit cards from your profile does send a red flag to PayPal, especially if you're doing it that many times in such a short period of time (i.e. 1 Day). I am working on a project that will sell anonymous offshore based stored value debit cards in mass via a very large private affiliate group.
