load up GTA IV like normal then hit start and go to game then new and it will give you the option of starting a new game in TBOGT.

That's great that you can get it but, what's the point? You can't use it to pick up anything, it's just a giant chopper with no guns.

Yeah, vehicles you bring yourself disappear, - but one ordered from Henrique usually stays ;) But exiting the original and entering the new one also takes time so the gain is limited. Another option is to drive a fast car to the mission start point and trigger the replay while in the car, - it will remove your original car but the 1st one driving along on the street is very likely the same type ...

Rumors aside, I don't think that the current GTA will get any more DLC. My guess is that all available help has been shifted to finishing the LA Noir and Agent projects. With a small planning contingent laying down the new playing field for GTA:Europe (my guess), GTA:Canada, GTA:North Pole or where ever they decide to throw the the next GTA protagonist.
