Its not entirely clear as to whether you have actually run scanpst.exe on the outlook data file. Does it work when you start with a completely new distribution list?
Another way to go would be to use categories to group your contacts. It is
much more reliable and gives your greater flexibility.

You can with Outlook 2007 or later. For Outlook 2003, you can try a
screenshot or choose;
File-> Save as Web Page...
You can then zip the files and send that.
Another way to go would be to export the calendar to a vcs-file.

Delegation permissions in not the same as having access permissions. For
one, you don't need to have the other. Make sure the permissions on the
folders have been set correctly and also check the root folder. The PA must
have at least Folder Visible permissions for that.
Thinks might be different for Scalix though. In the case, check with them
for support.

You can't import dbx files. You can only import from an installation of
Windows Mail.
File > Import and Export> Import from another Program or file > Windows
Mail.

utility for the mbx format.

The correct way is to move the . pst file. It contains all pertinent
information. I personally place the pst in a folder under (My) Documents so
that it is backed up during my normal back up.

they are copied to a local file. It's either a nst or ost format, depending
on who knows what. (I have both nst and ost for hotmail/live/msn acct. This
month the connector is using nst extension.)
