Hoju I sent you a pm but I guess I can feel stupid...If a child has H1N1 and an ear infection, can those ear temp checks where the tip is not cleaned spread the virus?? cause kids at my schools are dropping like flies and they all ear check.

then don't slip into friendly mode. you are her teacher, not her friend. you don't owe her friendship. be hard on her. if it is the only thing that works then continue. hopefully, after spending enough classes kneeling in a stress position, she'll come around, if not, so be it, the ball is in her court.

Do what I did when I worked in a high school (may not work for middle school) which is to make an example of them. If they act like disrespectful cunts, take their desk and put it in the hallway with them following, show them that you wont tolerate that kind of bs in your class. Shame them. After putting them and their desk in the hallway, tell the class why you did it and how the next step is to...

Caring professions are difficult for this... you just need to meditate a bit on what "professional distance" means. Don't overextend yourself any more than your students do or you will be... well... overextended!
