I don't know what the internal's of a heater box on a truck look like, however on my 78 van the heat/defrost was controlled by a rotary control about 2-1/2" in dia that was sealed with foam type weatherstrip that came apart more than 10 years ago. This caused similar problems.

Most likely the
kickdown bad strut (internal) has fallen out due to loose adjustment. It will require dropping the pan and valvebody to get to it and reinstall it.

Thats a job that I would like to do at one point also. My bet is that its not too bad a job, being able to take some measurements with the seat still in the donor truck would help.

I am not an auto guy, but it sounds as if your
torque converter isn't working correctly. Does it slip under load? I think I would change the fluid and screen just to rule it out as a first step.

I had same 90 ram, same issue. Mechanic said not to worry, it's a fairly common issue(IIRC it's bad rear trans seal) and his dads
ramcharger has been like that for 4years.I drove mine another50k (work/farm/hunt /cold)till rest of truck was shot, had no problems. Take a little more care leveling truck before checking ...
